CHECK-IN
We are excited to serve you at Nervana Medical. The following are important instructions to prepare for your appointment. Please give us a call if you need help or have any questions!
1 – It is important to first create an account with OnPatient, our patient portal and medical record system. You will use this portal to share important medical history, check-in to appointments, and review notes from your medical provider(s). To connect with your patient portal, look for an email with subject “Connect with Nervana Medical via onpatient”, then click “Sign up now”. If you can’t find the email, be sure to search for the subject line above and also check your junk folder. If you have already signed up, skip to step 3.
2 – When you sign up you will be asked to confirm your date of birth and phone number. Be sure to put exactly what was provided to Nervana Medical otherwise we won’t be able to connect to your account.
3 – Once your account is created, look for your appointment in the appointments tab. If you don’t see an appointment you might need to wait a few minutes and try again. Sometimes it will take a few minutes to create the connection to Nervana if your account was just created.
4 – Once you found your appointment, click “Check-in”. This will take you through a series of IMPORTANT medical history questions and have you sign the consents necessary for your appointment. Please answer with as much detail as possible. These questions help our providers to make the best clinical decisions possible for your treatment.
5 – You will be asked a variety of demographic and medical questions. The required fields are marked in red. Be sure to answer with detail so your provider has the information they need to best serve you. The last step is to read and sign the consent forms.
6 – After you have finished checking in, be sure to refer back to the email we have sent you for service specific requirements and information about your appointment. We look forward to seeing you soon!
