Thyroid optimization is the process of balancing your thyroid hormones to improve energy, metabolism, and overall well-being. Did you know that the thyroid affects nearly every part of your body? If it’s out of balance, it can have a negative impact on energy levels, weight, mood, and even your hair and skin. Nervana Medical provides personalized thyroid hormone replacement therapy to help restore your body’s natural balance and improve quality of life.
We also combine this with our Hormone Therapy services to support overall hormonal health. Learn more about how our experts can help you achieve optimal wellness through comprehensive hormone management.
Do You Have Thyroid Imbalance?
Most people are not actively thinking about their thyroid gland, but it’s a small butterfly-shaped organ in your neck. Imbalances occur when it produces either too little or or too much of vital hormones. This imbalance disrupts your body’s metabolism and can slow down or speed up essential body functions. Some of the hallmark symptoms of a possible thyroid imbalance can include;
- Fatigue or low energy
- Weight gain or difficulty losing weight
- Hair loss or thinning
- Feeling cold easily
- Brain fog or mood changes
- Irregular periods or low libido
If you checked any of these, thyroid optimization may help because it means your body is craving balance. Nervana Medical uses a personalized approach to thyroid hormone replacement therapy to address the root causes of these symptoms, moving beyond standard treatment to find your optimal levels.
Hypothyroidism vs. Hyperthyroidism
Thyroid optimization can help whether you have hypothyroidism or hyperthyroidism. Hypothyroidism essentially means the thyroid is underactive and not making enough hormones. Conversely, hyperthyroidism means your thyroid is overactive, making too many hormones. This can speed up the body’s functions. Consider the table below that breaks down the key differences:

FAQs About Thyroid Hormone Replacement Therapy
What is thyroid hormone replacement therapy?
It’s a treatment that balances thyroid hormone levels to improve energy, metabolism, and overall health.
How do I know if I need thyroid optimization?
Common signs include fatigue, weight changes, hair loss, brain fog, feeling cold, and menstrual irregularities.
Is thyroid hormone replacement therapy safe?
Yes, when prescribed and monitored by medical professionals, it’s safe and effective.
How long does it take to feel better?
Most patients notice improvements within a few weeks, but results can vary.
What do these labs mean anyways?
- TSH (Thyroid Stimulating Hormone): A brain signal that tells the thyroid to produce more hormone.
- Free T4: Storage form; converts to T3.
- Free T3: Active hormone that drives metabolism, energy, and mental clarity.
- Reverse T3: Inactive form that can block T3 action under stress or inflammation.
- Thyroid Antibodies (TPO, TgAb): Identify autoimmune thyroid issues like Hashimoto’s.
Optimizing thyroid health often means aiming for:
- TSH: around 0.5–2.0 µIU/mL (depending on symptoms)
- Free T3: upper half of normal range
- Free T4: mid-to-upper range
What are the different types of treatment options available for people with hypothyroidism?
- Levothyroxine (T4 replacement)
- Brand names: Synthroid, Levoxyl, Unithroid, Euthyrox, Tirosint.
- Mechanism: Synthetic version of thyroxine (T4), which the body converts to the active T3 hormone.
Most common use: First-line for hypothyroidism; steady, long-acting. - Notes: Best taken on an empty stomach; absorption affected by calcium, iron, and some foods.
- Liothyronine (T3 replacement)
- Brand name: Cytomel.
- Mechanism: Synthetic triiodothyronine (T3), the active form of thyroid hormone.
- Use: Occasionally used when patients don’t convert T4 to T3 efficiently, or in combination with T4.
Notes: Shorter half-life; can cause palpitations or anxiety if dosed too high.
- Combination T4/T3 therapy
- Examples:
- Desiccated thyroid extract (DTE): Armour Thyroid, NP Thyroid, Nature-Throid.
Synthetic combos: Custom-compounded T4/T3 capsules.
- Desiccated thyroid extract (DTE): Armour Thyroid, NP Thyroid, Nature-Throid.
Mechanism: Derived from porcine thyroid or compounded to mimic natural ratios of T4 and T3.
Pros: Some patients report better energy and mood. What is naturally-derived* desiccated thyroid treatment and what is the main source for NP Thyroid®?
NP Thyroid® is a desiccated thyroid extract (DTE) therapy containing thyroxine (T4) and triiodothyronine (T3) in one pill. NP Thyroid® is naturally-derived* from dried porcine thyroid glands.
*Naturally-derived refers to the biological nature of the porcine-derived desiccated thyroid ingredient found in the product.
Is NP Thyroid® gluten-free?
NP Thyroid® contains no ingredient made from a gluten-containing grain (wheat, barley or rye).1
What are all the ingredients in NP Thyroid®?
The active ingredient in NP Thyroid® is desiccated thyroid extract (DTE), which contains both thyroxine (T4) and T3 (triiodothyronine). Inactive ingredients include calcium stearate, dextrose, and mineral oil.
How long before it starts to work

*Patients switching from NP to Synthroid may feel a temporary dip in energy for 2–4 weeks as the body adjusts and builds up T4 stores.
Lab monitoring:Wait at least 6 weeks after a dose change before checking TSH or Free T4, since that’s when steady state is achieved.
Symptom tracking: We encourage patients to track fatigue, mood, weight, and temperature tolerance during this adjustment period.
How do I take the medication for Best Efficacy
- Take It on an Empty Stomach and be Consistent
- Take thyroid medication first thing in the morning, ideally 30–60 minutes before eating or drinking anything (other than water).
- Food;especially coffee, milk, high-fiber foods, and calcium-containing products, significantly decreases absorption.
- Be Consistent With Timing; Take it at the same time every day.The circadian rhythm of thyroid hormone levels benefits from consistency. Small timing changes (within 1 hour) are fine; large daily fluctuations can affect lab results and symptoms.
What is NP Thyroid® used for?
NP Thyroid® (thyroid tablets, USP) is indicated:
- As a replacement or supplemental therapy in patients with hypothyroidism of any etiology, except transient hypothyroidism during the recovery phase of subacute thyroiditis. This category includes cretinism, myxedema, and ordinary hypothyroidism in patients of any age (children, adults, the elderly), or state (including pregnancy); primary hypothyroidism resulting from functional deficiency, primary atrophy, partial or total absence of thyroid gland, or the effects of surgery, radiation, or drugs, with or without the presence of goiter; and secondary (pituitary), or tertiary (hypothalamic) hypothyroidism
- As a pituitary thyroid-stimulating hormone (TSH) suppressant, in the treatment or prevention of various types of euthyroid goiters, including thyroid nodules, subacute or chronic Iymphocytic thyroiditis (Hashimoto’s), multinodular goiter, and in the management of thyroid cancer
