$150/month
- Up to 4 same or next‑day appointments per month (in‑person or telehealth)
- Management of common primary & urgent care concerns (UTI, cold sores, high blood pressure, etc.) and functional health requests (B12, NAD, methylene blue, glutathione)
- Annual comprehensive wellness consult – holistic review of every system from metabolic to mental health
- Consults for all Nervana services without continued management
- Medications, labs, and shipping are billed separately

- Up to 2 same or next‑day appointments per month (in‑person or telehealth)
- Weight-loss program management, including GLP-1/GIP medications (semaglutide, tirzepatide), nutrition guidance, and metabolic testing
- Medications typically cost $90–$300/month; prescriptions may be sent to compounding pharmacy or retail pharmacy per provider discretion
- Access to 1,000 mg NAD for under $100/month at compounding pharmacies while enrolled
