What the Research Shows

One study published in Lasers in Medical Science compared women with idiopathic hirsutism to women with PCOS after six long-pulsed Nd:YAG laser treatments.
The results were significant.
Approximately 70 percent of women without a hormonal disorder achieved greater than 75 percent hair reduction after six treatments. Among women with PCOS, that number was 54 percent.
That does not mean laser hair removal is ineffective for PCOS.
It means hormonal stimulation continues after treatment, allowing new follicles to become active over time. This distinction is important because it changes how treatment should be planned and how results should be measured.
For many women with PCOS, success comes from combining effective laser treatments with a long-term management plan rather than expecting a permanent one-time solution.
How Laser Hair Removal Works
Laser hair removal uses a principle called selective photothermolysis.
The laser delivers energy that is absorbed by melanin within the hair shaft. That energy converts to heat, damaging the follicle while minimizing injury to the surrounding skin.
Hair grows in cycles, and only follicles that are actively growing contain enough pigment to respond effectively to treatment. Because not every hair is in the same growth phase at the same time, multiple sessions are always necessary.
This is true whether someone has PCOS or not.
Hormonal conditions simply increase the likelihood that additional follicles will enter an active growth phase over time.

For some patients, adding topical Eflornithine 13.9% cream can improve overall results.
Eflornithine works by inhibiting ornithine decarboxylase, an enzyme involved in hair follicle cell division. As cellular activity slows, hair grows more slowly and often becomes finer and less noticeable.
Most patients begin seeing improvement after six to eight weeks of consistent twice-daily use.
To maximize absorption, the treated area should not be washed for at least four hours after application.
Because Eflornithine works through a completely different mechanism than laser hair removal, the two treatments complement one another rather than compete.
Why Combining Treatments Produces Better Results
For women with PCOS and other causes of hormonal facial hair, laser hair removal is often most effective when it is part of a broader treatment plan.
Laser treatment and Eflornithine work in different ways. The laser damages actively growing hair follicles by targeting melanin within the hair shaft. Eflornithine slows the cellular activity that allows hair to grow. Because these treatments target different parts of the hair growth process, they can work together to improve overall results.
This combination has been studied in clinical trials.
In a randomized study published in the Journal of the American Academy of Dermatology, researchers compared laser hair removal alone with laser hair removal combined with topical Eflornithine. The combination group achieved complete or near-complete hair removal in 93.5 percent of treated areas, compared with 67.9 percent in the laser-only group.
Patients using both treatments also experienced faster improvement and longer periods before noticeable regrowth occurred.

Preparing for Laser Hair Removal
Proper preparation helps improve both safety and results.
One of the biggest misconceptions is that you should avoid shaving before laser hair removal. In reality, shaving is recommended because it leaves the hair root in place while removing excess hair above the skin’s surface.
The laser targets pigment within the follicle below the skin. Long hairs sitting above the skin absorb unnecessary energy and can increase discomfort during treatment.

Shave the Treatment Area
Shave approximately 24 hours before your appointment unless instructed otherwise by your provider.
Avoid Waxing or Plucking
Do not wax, tweeze, thread, or use an epilator for at least four weeks before treatment. These methods remove the hair root that the laser is designed to target.
Limit Sun Exposure
Avoid excessive sun exposure and self-tanning products for approximately two weeks before treatment. Recently tanned skin contains more melanin, which increases the risk of unwanted pigment changes after treatment.

What to Expect During Treatment
Laser hair removal is a quick outpatient procedure that requires little to no downtime.
Most facial treatments take only a few minutes to complete.
Patients often describe the sensation as similar to a rubber band snapping against the skin, although comfort varies depending on the treatment area and individual pain tolerance.
Immediately after treatment, mild redness and swelling around the hair follicles are common. These effects usually resolve within several hours.
Over the following one to three weeks, treated hairs gradually shed from the follicles. This is a normal part of the treatment process and is often mistaken for continued hair growth.
Because only actively growing hairs respond to treatment, additional sessions are scheduled several weeks apart to target follicles as they enter their next growth cycle.
Setting Realistic Expectations
