Tips & Tricks for Retinol / Retinoic Acid Compliance and Layering (The Buffer Methods)

At Nervana Medical in Sandy, Utah, we know that retinoids, whether over-the-counter retinol or prescription-strength tretinoin; are some of the most powerful tools in modern acne and anti-aging skincare. However, their incredible benefits often come with an adjustment period marked by dryness, redness, and irritation. That’s why we focus on personalized strategies to improve retinoid tolerance and long-term compliance, so our patients can experience smoother texture, clearer pores, and brighter, more youthful skin without giving up halfway through the “retinization” phase. Understanding proper layering and buffering methods can make all the difference between frustration and flawless results.

The key to long-term compliance is managing the common side effects of irritation, dryness, and redness. Layering with a moisturizer is the most effective way to “buffer” the retinoid’s effects without sacrificing its long-term benefits.

1. The “Sandwich Method” (Maximum Buffering)

This is the recommended method for beginners, sensitive skin, or when using a high-strength product like Tretinoin. It dilutes the concentration slightly upon application and prevents moisture loss.

  • Step 1 (Bread): Apply a thin layer of a gentle, non-occlusive moisturizer to clean, dry skin.
  • Step 2 (Filling): Apply a pea-sized amount of your retinoid over the moisturizer.
  • Step 3 (Bread): Apply a second, generous layer of the moisturizer to seal everything in.

2. The “Buffer Method” (Moisturizer First)

A slightly less buffered approach, good for intermediate users.

  • Step 1: Apply your moisturizer to clean, dry skin.
  • Step 2: Wait about 10-20 minutes for the moisturizer to fully absorb.
  • Step 3: Apply a pea-sized amount of the retinoid.

General Compliance Tips

  • Start Slow: Begin with a low concentration and apply it only 1-2 nights per week. Gradually increase to every other night, and then nightly, only as your skin tolerates it. Consistency over time is more important than speed.
  • Apply to DRY Skin: Always ensure your skin is completely dry (wait at least 20 minutes after cleansing) before applying your retinoid. Applying to damp skin can increase absorption too rapidly, leading to significant irritation and stinging.
  • Less is More: Use only a pea-sized amount for your entire face. Using more product does not mean faster results; it only increases the risk of irritation.
  • Avoid Combining Actives: On the nights you use your retinoid, simplify your routine. Do not use (or use caution/ follow your provider advice)  other harsh actives such as:
    • Alpha Hydroxy Acids (AHAs) like Glycolic or Lactic Acid.
    • Beta Hydroxy Acids (BHAs) like Salicylic Acid.
    • Benzoyl Peroxide. If using benzoyl peroxide (BPO) too, apply BPO AM and retinoid PM (BPO can inactivate some tretinoin formulations).
    • High-concentration Vitamin C (generally, use Vitamin C in the morning, not at night).
  • Sunscreen is Non-Negotiable: Retinoids increase photosensitivity. You must use a broad-spectrum sunscreen with an SPF of 30 or higher every morning, regardless of the weather.

With the right approach, retinoids can completely transform your skin’s health and appearance; minimizing breakouts, softening fine lines, and improving tone and texture over time.
